Study Notes
Hepatic Cirrhosis
- Chronic disease replacing normal liver tissue with fibrous scar tissue
- Disrupts liver structure and function
- Three main types:
- Alcoholic cirrhosis: Scar tissue surrounds portal areas, usually from chronic alcoholism
- Post-necrotic cirrhosis: Wide bands of scar tissue resulting from previous acute viral hepatitis
- Biliary cirrhosis: Scarring around bile ducts, often due to chronic biliary obstruction or cholangitis; less common
Pathophysiology of Cirrhosis
- Several factors involved, but alcohol consumption is the major causative factor
Clinical Manifestations of Cirrhosis
- Intermittent mild fever
- Unexplained nosebleeds (epistaxis)
- Ankle swelling (edema)
- Indigestion, especially in the morning
- Gas and bloating (flatulent dyspepsia)
- Other possible manifestations include:
- Abdominal pain
- Enlarged, firm liver
- Enlarged spleen
- Ascites (fluid buildup in the abdomen)
- Jaundice (yellowing of skin and eyes)
- Weakness
- Weight loss
- Persistent mild fever
- Low blood pressure (hypotension)
Diagnostic Tests for Cirrhosis
- Serum albumin levels decrease, while globulin levels increase
- Liver enzyme tests indicate liver cell damage (e.g., serum alkaline phosphatase)
- Bilirubin tests assess bile excretion or retention
- Prothrombin time is prolonged
- Ultrasound scans assess differences in density between healthy and scarred tissue
- Other diagnostic tests may include CT scan or MRI
Nursing Care (Similar to pancreatitis care)
- No specific nursing care details are provided, only stating that the care is similar to pancreatitis care.
