Healthy Pregnancy Lecture
- Conception is the union of sperm from the male and egg from the female.
- Term pregnancy lasts 40 weeks.
Human Development
- Preembryonic Stage (fertilization- 14 days):
- Implantation in uterine lining.
- Cellular multiplication and differentiation.
- Yolk sac develops by day 8/9, providing nutrients for cell growth.
- Embryonic Development (day 15-8 weeks):
- Structural changes, including the formation of arm and leg buds and differentiation of the brain.
- Heart develops four chambers by day 49.
- Cardiac structure begins to beat by day 23.
- Fetal Development (9 weeks-birth):
- Development of all organs and structures.
- Genitals are differentiated
- Urine production, reflexes and brain development start by 9 weeks
- Nails and lanugo (thin peach fuzz) develop by 20 weeks.
- Fetal movements felt by the mother during this period.
- Skin covered with vernix (protective coating).
- Lungs maturing, allowing for (some) gas exchange. -Development of neuro system by week 25. -Third trimester: Continued system development and fat storage.
Teratogens
- Agents that affect the embryo during pregnancy.
- Most vulnerable time to teratogens is between 2-8 weeks gestation (embryonic period).
- Illicit drug use and alcohol consumption are harmful during pregnancy.
Fertilization-12 weeks
- Pregnancy is divided into 3 trimesters (13 wk increments).
- Rapid changes occur during the first 12-13 weeks.
- By week 12, the baby resembles a developing human.
Anatomy
- Placenta:
- Organ that develops in the uterine wall.
- Allows oxygen and nutrients to reach the fetus, and waste removal.
- Blood flow to and from the placenta is through the umbilical cord.
- Amniotic Fluid:
- Fluid surrounding the baby.
- Provides cushioning and protection to the developing fetus.
- Umbilical Cord:
- Connects the fetus to the placenta.
- Carries oxygen and nutrients to the fetus and removes waste products.
Formation of Twins
- Monozygotic twins come from one fertilized egg.
- Dizygotic twins are from two separate eggs.
- Monozygotic twins are genetically identical.
- Dizygotic twins are no more similar than siblings.
Maternal Changes R/T Pregnancy
- Uterus: increases in size, going from pear/fist size to underneath the breast; thickness due to hormones
- Breasts: get larger and more tender
- Cervix: pigmentation can become darker, increased vascularization, softening of the cervix.
- Vagina: increased connective tissue, acidic secretions.
- Cardiovascular: increased cardiac output peaks by 30 weeks.
Labor and Delivery Terminology
- Preterm or premature labor occurs before 37 weeks.
- Postterm labor occurs after 42 weeks.
- Stillbirth is a delivery of a fetus over 20 weeks with no heartbeat.
