- Problem-Oriented Health Records (POR): Focus on patient problems, each assigned a unique number. The record includes a database of patient information, an initial care plan, and progress notes using the SOAP format.
- Integrated Health Records: Documentation from different sources is combined in chronological order. This makes it easier to follow the patient's treatment but can hinder comparing similar information.
SOAP format
- Subjective Observations: Patient's complaints, symptoms, and experiences.
- Objective Observations: Measurable findings such as vital signs, lab results, and physical examinations.
- Assessment: Current situation using the information from Subjective and Objective observations.
- Plan: Future action based on the assessment and includes recommendations, prescriptions, additional tests, and referrals.
Health Record Numbering and Filing systems
- Serial Numbering and Filing: Each new treatment creates a new record.
- Unit Numbering and Filing: Patients keep the same record number for all treatments.
- Serial-Unit Numbering Filing: Combines both systems, assigning a new number while retaining access to past records.
- Straight Numeric Filing: Arranges records in numerical order.
- Terminal Digit Filing: Reads numbers right to left, organizing by the last two digits first.
- Middle Digit Filing: Similar to terminal digit filing, but the middle digits take precedence.
