Mishap Prevention
- Effective mishap prevention relies on prompt and efficient identification, reporting, and correction of hazards by personnel.
AF Form 457
- AF Form 457 is the USAF Hazard Report (HR).
- Commanders must ensure availability of AF Form 457 to all personnel for hazard reporting.
Hazard Reporting
- Any Air Force personnel, including those under contract, can report a hazard.
- Reports can be made anonymously.
Immediate Action for Hazards
- If a hazard poses critical or imminent danger, the responsible supervisor must take immediate action to mitigate or eliminate it to protect personnel or property.
Investigation Responsibility

- HRs for critical/imminent danger situations are investigated within 1 duty day.

Reporting and Monitoring Actions
- The responsible individual must complete Part II, "Action Taken," within 10 working days and return the HR to the safety office for monitoring.
- Investigators must inform the report originator (if known) about the completed action within 10 workdays after the report is resolved.
Reporting for Transient Personnel
- Transient personnel unable to report a hazard where it is found should submit the HR at the next Air Force base they visit or to their home base safety office.
Weather-Related Hazards
- Individuals identifying weather forecasting hazards should submit reports as soon as possible to preserve records.
Hazard Reports Involving Other Entities
- Installation safety staffs must send hazard reports related to other military services, foreign nations, or outside agencies to HQ AFSEC/SE (Air Force Safety Center, Kirtland NM).
Higher Level Appeals
- Higher-level appeal responses must be sent to the employee within 20 calendar days.
- Employees can appeal to the next higher level if they do not receive a response or are dissatisfied with the reply within 20 calendar days.
