Glycopyrrolate Overview
- Glycopyrrolate is classified as an anticholinergic medication.
- Commonly supplied in concentrations of 0.2 mg/mL, available in vial sizes of 1mL, 2mL, 5mL, and 20mL.
Actions of Glycopyrrolate
- Inhibits acetylcholine's muscarinic effects on autonomic system targets innervated by post-ganglionic cholinergic nerves.
- Blocks cardiac vagal reflexes by preventing vagal inhibition at the sinoatrial (SA) node.
Indications
- Used to counteract cholinergic side effects caused by anticholinesterases in neuromuscular blockade reversal.
- Administered preoperatively to reduce secretions and manage cardiac vagal reflexes.
Dosing Information
- For blocking cholinergic effects, the recommended dose is 0.2 mg IV per 1 mg of neostigmine administered IV/IO.
- Adult preoperative dosage is 0.0044 mg/kg intramuscularly, typically given 30-60 minutes prior to surgery.
Contraindications
- Not to be used in patients with hypersensitivity to the drug.
- Contraindicated in individuals with glaucoma, obstructive uropathy, obstructive gastrointestinal conditions, myasthenia gravis, and those with unstable cardiovascular status during acute hemorrhage.
Precautions
- Use with caution in patients with coronary artery disease (CAD), arrhythmias, and heart failure.
- Monitor blood pressure in patients with hypertension.
- Caution is warranted in patients with hepatic and renal diseases.
