Study Notes
- Prof. Dr. Abdullah Bahnacy discusses functional GIT disorders, specifically focusing on Globus and functional chest pain.
- Globus is characterized by a persistent or intermittent sensation of a lump or foreign body in the throat. Occurs between meals with no dysphagia or pain on swallowing. Treatment includes explanation and reassurance, anti-reflux therapy, and antidepressants.
- Functional chest pain presents as midline chest pain, not burning in nature, potentially of esophageal origin. More than half of patients respond to high-dose acid suppression therapy in the first week, with some responding to nitrates and calcium-channel blockers. A history of psychiatric disorder is found in over 60% of patients, with antidepressant therapy, such as amitriptyline, being used.
- Functional dyspepsia is the second most common functional gastrointestinal disorder, affecting patients with a spectrum of symptoms including upper abdominal pain/discomfort, fullness, early satiety, bloating, and nausea. Patients have no structural abnormality explaining their symptoms.
- Functional dyspepsia has two subgroups: epigastric pain syndrome, characterized by pain centred in the upper abdomen as the predominant symptom.
