    Study Notes

    Deep Perineal Pouch in Female

    • Contains part of the urethra and vagina
    • Includes the sphincter urethrae, pierced by the urethra and vagina
    • Houses deep transverse perineal muscles
    • Contains internal pudendal vessels and their branches
    • Includes dorsal nerves of the clitoris

    Female Urethra

    • Approximately 1 1/2 inches (3.8 cm) long
    • Extends from the neck of the bladder to the external meatus
    • Opens into the vestibule about 1 inch (2.5 cm) below the clitoris
    • Traverses the sphincter urethrae and lies in front of the vagina
    • Small openings of the ducts of the paraurethral glands are located at the sides of the external urethral meatus

    Paraurethral Glands

    • Correspond to the prostate in the male
    • Open into the vestibule by small ducts on either side of the urethral orifice

    Greater Vestibular Glands

    • A pair of small mucus-secreting glands
    • Located under the posterior parts of the bulb of the vestibule and the labia majora
    • Each drains its secretion into the vestibule by a small duct
    • Ducts open into the groove between the hymen and the posterior part of the labium minus

    Vagina

    • Extends upward and backward between the vulva and the uterus
    • Approximately 3 inches (8 cm) long
    • Cervix of the uterus pierces its anterior wall
    • Vaginal orifice in a virgin possesses a thin mucosal fold called the hymen
    • Upper half of the vagina lies within the pelvis, lower half lies within the perineum
    • Supports: upper third (levatores ani muscles, transverse cervical, pubocervical, and sacrocervical ligaments), middle third (urogenital diaphragm), lower third (perineal body)
    • Blood supply: vaginal artery (branch of internal iliac artery) and vaginal branch of uterine artery
    • Veins: vaginal veins drain into internal iliac veins
    • Lymph drainage: upper third (internal and external iliac nodes), middle third (internal iliac nodes), lower third (superficial inguinal nodes)
    • Nerve supply: nerves from inferior hypogastric plexuses

    Vulva

    • Collective name for the female external genitalia
    • Includes mons pubis, labia majora and minora, clitoris, vestibule of vagina, vestibular bulb, and greater vestibular glands
    • Lymph drainage: skin of vulva is drained into medial group of superficial inguinal nodes
    • Nerve supply: anterior parts (ilioinguinal nerves and genital branch of genitofemoral nerves), posterior parts (branches of perineal nerves and posterior cutaneous nerves of the thigh)
    • Blood supply: branches of external and internal pudendal arteries on each side
