Study Notes
Overview of Famotidine
- Famotidine, commonly known as Pepcid, is an antiulcer medication.
- It is classified as an H2 blocker, which reduces gastric acid secretion.
Mechanism of Action
- Inhibits H2 receptors on gastric parietal cells.
- Decreases gastric secretions to alleviate conditions such as ulcers and GERD.
Uses
- Treats gastric and duodenal ulcers, pyrosis, and dyspepsia.
- Prophylaxis for stress ulcers and management of Zollinger-Ellison syndrome.
Adverse Effects
- Possible adverse effects include palpitations, dysrhythmias, severe allergic reactions, and blood dyscrasias.
Contraindications
- Contraindicated in patients with hypersensitivity, pregnancy, breastfeeding, renal or hepatic disease, and certain population groups like older adults and children.
Side Effects
- Common side effects encompass headaches, dizziness, drowsiness, confusion, and gastrointestinal issues (nausea/vomiting, diarrhea, constipation).
- Other side effects may include depression, arthralgia, myalgia, weakness, erectile dysfunction, and gynecomastia.
Administration Routes
- Available for oral (PO) and intravenous (IV) administration.
Onset Times
- Oral Famotidine has an onset of approximately 1 hour.
- Intravenous administration onset is between 10 to 15 minutes.
Drug Interactions
- Reduces absorption of iron and efficacy of certain medications like ketoconazole and naproxen.
- May increase action of metformin and elevate serum alkaline phosphatase levels.
Assessment and Monitoring
- Assess pain characteristics (type, duration, severity, frequency, location).
- Monitor fluid and electrolyte balance, gastric pH (>5), blood urea nitrogen (BUN), creatinine, and drug history.
Nursing Diagnoses and Care Planning
- Nursing diagnosis includes acute pain related to excessive gastric secretion.
- Goal: Decrease abdominal pain within 1 to 2 weeks of therapy.
Nursing Interventions
- Adjust dosage for older adults to prevent metabolic acidosis.
- Educate patients on medication adherence, smoking cessation, and the separation of Famotidine and iron doses by at least 1 hour.
- Inform about the timeframe (8 weeks) for peptic ulcer healing and provide advice on driving and potential erectile dysfunction.
Patient Education
- Encourage reporting concerning symptoms such as pain and bleeding.
- Highlight the importance of diet, avoiding spicy, acidic, and fatty foods to prevent gastric irritation.
Relaxation Techniques
- Suggest deep breathing exercises, progressive muscle relaxation, and guided imagery to manage anxiety.
Importance of B12
- Emphasize the necessity of foods high in vitamin B12 to prevent deficiency and support nerve function.
Evaluation Criteria
- Progress is measured by absence of pain and evidence of healing.
