- A planned, ongoing, and purposeful activity where the client and healthcare professional determine:
- Client progress towards goal/outcome achievement
- Effectiveness of the nursing care plan
- Happens continuously, done while or immediately after implementing nursing orders

- Evaluation depends on effective preceding phases
