Study Notes
Brachial Plexus Anatomy
- The brachial plexus is a network of nerve fibers supplying the upper limb, originating in the neck and extending through the axilla to the arm.
- Comprises the anterior rami of cervical spinal nerves C5, C6, C7, C8, and thoracic nerve T1.
- Roots of the plexus are formed by spinal nerves C5-T1, which separate into anterior and posterior rami.
- The roots pass between the anterior and medial scalene muscles to enter the neck.
Trunks and Divisions
- At the neck, the roots converge into three trunks:
- Superior trunk: C5 and C6
- Middle trunk: C7
- Inferior trunk: C8 and T1
- Each trunk divides into anterior and posterior branches within the posterior triangle of the neck, leading to major cords in the axilla.
Cords and Major Branches
- Cords are formed in the axilla relative to the axillary artery:
- Lateral cord: Anterior divisions of superior and middle trunks
- Posterior cord: Posterior divisions of all trunks
- Medial cord: Anterior division of the inferior trunk
- Major branches include Musculocutaneous, Axillary, Median, Radial, and Ulnar nerves.
Erb's Palsy
- Erb’s palsy is characterized by muscle weakness in the arm due to nerve injury, prevalent in infants during birth.
- Common causes include shoulder traction during delivery, motor vehicle accidents, and traumatic injuries (gunshot, industrial, sports-related).
- Risk factors involve excessive maternal weight gain, diabetes, prolonged labor, large infant size, and abnormal pelvis shape.
Signs and Symptoms of Erb's Palsy
- Lack of active movement in the affected arm.
- Arm typically hangs limp, adducted, and internally rotated.
- Shoulder shows decreased external rotation; subluxation may occur.
- "Waiter's tip" posture is a characteristic sign.
Diagnosis
- Conducted via patient history, physical examination, and neurological testing.
- Imaging studies such as X-rays, MRI, and CT scans may be utilized.
- Electromyography (EMG) and nerve conduction studies (NCS) are essential for evaluation.
Treatment Options
- Physiotherapy and hydrotherapy play crucial roles in recovery.
- Surgical intervention may be necessary for severe cases.
- Occupational therapy aids in regaining functional use of the arm.
- Botulinum toxin injections may be employed to relieve muscle contractures.
