    Study Notes

    Embryology of the Tooth Root

    • After the bell stage, the tissues of the tooth root and supporting structures start to form
    • The processes involved include: dentinogenesis, pulp formation, cementogenesis, periodontal ligament formation, and osteogenesis.
    • These processes form the tissues of the root and supporting structures, including root dentine, radicular pulp, cementum, periodontal ligament, and alveolar bone.
    • The formation of the root starts at the base of the crown, at the site of the future cemento-enamel junction (CEJ) and continues to the apical foramen.
    • The process continues even after active tooth eruption is complete.

    Hertwig's Epithelial Root Sheath (HERS)

    • HERS forms from the cervical loop and determines the shape of the root.
    • HERS consists of inner and outer enamel epithelium, but it doesn't differentiate further.
    • It surrounds the dental follicle (sac) and encompasses the dental papilla as pulp continues to form.
    • HERS creates infolds for multiple roots as it extends to the apical foramen.

    Dentinogenesis of the Root

    • HERS signals the continuation of dentinogenesis.
    • Odontoblasts continue to form dentine along the root sheath, consistent with the crown.
    • This process continues to the apical foramen and is the same as in the crown.
    • Radicular pulp forms from the dental papilla within the root canals.

    Formation of Supporting Structures

    • After dentinogenesis of the root starts, the pre-dentine contacts and triggers HERS to disintegrate.
    • Cells of the dental sac contact the pre-dentine, initiating the differentiation of cementoblasts and fibroblasts to form cementum and the periodontal ligament.
    • Ectomesenchymal cells surrounding the dental sac differentiate into osteoblasts and osteoclasts to form alveolar bone similar to other bone development.

    Apical Foramen and Root Canals

    • The apical foramen forms at the base of the root (not necessarily in the center), providing a pathway for blood vessels, nerves, and lymph vessels to supply the pulp of the tooth.
    • Pulpal root canals are spaces where the radicular pulp continues to form in the root.
    • Initially, they are wide and open, gradually narrowing over time.
    • The wide-open canals partly explain why a young tooth may survive pulpal injury.

    Clinical Significance of Timing

    • Root formation continues after tooth eruption, highlighting the importance of root development for tooth function and stability in the oral cavity.

    Accessory Root Canals

    • If the continuity of HERS breaks before dentine forms along the root, adjacent odontoblasts cannot form dentine, resulting in a defect in the dentin tubule through to the pulp.
    • This creates an accessory root canal.
    • This has clinical significance for root canal treatment and periodontal treatment, potentially causing perio-endo lesions.

    Epithelial Rests of Malassez

    • Remnants of HERS remain adjacent to the root surface after disintegration, known as Epithelial Rests of Malassez.
    • They are found in the periodontal ligament.
    • They have been linked to periodontal cysts and may play a role in periodontal repair and regeneration.

    Anomalies: Enamel Pearls

    • Remnants of the inner enamel epithelium may differentiate into ameloblasts to form enamel pearls, particularly at the CEJ and in furcation areas.
    • They have clinical significance in treating periodontal disease.

Why Doesn't Enamel Form on the Tooth Root?

    • HERS consists of outer and inner enamel epithelium.
    • In crown embryology, ameloblasts differentiate from inner enamel epithelium, but this doesn’t happen in the root because ameloblasts are not present.
    • The layers present in HERS differ from those in the enamel organ, preventing enamel formation on the root.
