Application for Driver License
- Each application for a driver license must be made in a format designated by the department and sworn to or affirmed by the applicant.
- The application must include the following information:
- Full name, gender, proof of social security card number, county of residence, mailing address, proof of residential address, country of birth, and a brief description.
- Proof of birth date satisfactory to the department.
- Proof of identity satisfactory to the department, which can include:
- A driver license record or identification card from another jurisdiction.
- A certified copy of a United States birth certificate.
- A valid, unexpired United States passport.
- A naturalization certificate issued by the United States Department of Homeland Security.
- A valid, unexpired alien registration receipt card (green card).
- A Consular Report of Birth Abroad provided by the United States Department of State.
- An unexpired employment authorization card issued by the United States Department of Homeland Security.
- Proof of nonimmigrant classification provided by the United States Department of Homeland Security.
Additional Requirements
- The application must include a consent to release driving record information.
- The department may require applicants to produce United States Department of Homeland Security documents for the sole purpose of establishing the maintenance of, or efforts to maintain, continuous lawful presence.
- Each application may include fingerprints and other unique biometric means of identity.
- The application form for an original, renewal, or replacement driver license or identification card must include language permitting voluntary contributions to various organizations.

- Contributions may be made to the following organizations:
- Health Care Trust Fund for organ and tissue donor education and for maintaining the organ and tissue donor registry.
- Florida Council of the Blind.
- Hearing Research Institute, Incorporated.
- Juvenile Diabetes Foundation International.
- Children’s Hearing Help Fund.
- Family First, a nonprofit organization.
- Florida Heart Research Institute, a nonprofit organization.
- Senior Vision Services, which shall be distributed to the Florida Association of Agencies Serving the Blind, Inc., a not-for-profit organization.
- The Arc of Florida.
- Ronald McDonald House Charities of Tampa Bay, Inc.
- League Against Cancer/La Liga Contra el Cancer, a not-for-profit organization.
- Lauren’s Kids, Inc., a nonprofit organization.
- Prevent Blindness Florida, a not-for-profit organization.
- State homes for veterans, to be distributed on a quarterly basis by the department to the Operations and Maintenance Trust Fund within the Department of Veterans’ Affairs.
- Disabled American Veterans, Department of Florida, a nonprofit organization.
- Achievement and Rehabilitation Centers, Inc., Autism Services Fund.
- Support Our Troops, Inc., a Florida not-for-profit organization.
- The State Office on Homelessness.

Veterans' Benefits
- The application form for an original, a renewal, or a replacement driver license or identification card must include a voluntary checkoff authorizing a veteran of the United States Armed Forces to request written or electronic information on federal, state, and local benefits and services available to veterans.
- The requested information shall be delivered to the veteran by any third-party provider selected by the Department of Veterans’ Affairs to act on its behalf.
- The Department of Veterans’ Affairs shall disseminate the contact information for each veteran who requests information to the third-party provider acting on its behalf.
- The contact information of a veteran obtained by a third-party provider may be used only as authorized by this subsection.
