Study Notes

Dispatch and Initial Response

  • Respond to calls for unresponsive individuals, such as a female at the gym.
  • Bring an AED and be prepared to perform CPR upon arrival.

Assessing Responsiveness

  • Tap and shout to check for responsiveness.
  • If unresponsive, call for help immediately and proceed to check for breathing and pulse.

Rescue Breathing Protocol

  • If the patient has a pulse but is not breathing normally, deliver rescue breaths at a rate of one breath every 6 seconds (10 breaths per minute).
  • Check for pulse every 2 minutes and perform CPR if the pulse is absent.

Transition to CPR

  • If there is no pulse after 4 minutes of rescue breathing, immediately initiate CPR.

AED Use during CPR

  • When an AED arrives and indicates no shock is advised, continue CPR.
  • Maintain CPR until the AED prompts a rhythm check, ALS team arrives, or the patient shows signs of movement.

Checking Pulse in Infants

  • Use 2 or 3 fingers to palpate the brachial pulse located halfway between the shoulder and elbow.

Child and Infant CPR Guidelines

  • For children, chest compressions should be at a rate of 100-120 compressions per minute and a depth of 2 inches using one or two hands on the lower part of the breastbone.
  • For infants, the rate remains 100-120 compressions per minute with a compression depth of 1.5 inches, using two fingers just below the nipple line at the center.

Compression to Ventilation Ratios

  • The recommended ratio for one rescuer is 30 compressions to 2 breaths.
  • For two rescuers, the ratio is 15 compressions to 2 breaths for infants and children.

Pediatric BLS Steps

  • Ensure scene safety before approaching the child.
  • Assess responsiveness and activate emergency response if needed.
  • If the child is unresponsive with no normal breathing but has a pulse, provide rescue breaths every 2-3 seconds.

Case Scenario Responses

  • In a scenario with an unresponsive 6-year-old, verify safety, assess responsiveness, and check for breathing and pulse.
  • If the child shows no breathing but does have a pulse, deliver rescue breaths at a rate of 1 breath every 2-3 seconds (20-30 breaths per minute).

AED Use Following a Shock

  • After an AED delivers a shock, perform CPR with a compression to breath ratio of 30:2.

Infant Response Protocol

  • In a case involving an unconscious infant, ensure the scene is safe, assess the infant’s responsiveness, and check for breathing for 5-10 seconds while feeling the brachial pulse.
