Difficult Airway Algorithm
- Difficult airway predicted: If a difficult airway is predicted, the first step is calling for assistance
- Forced to act?: if forced to act, next give RSI drugs, if not, continue
- Failure to maintain oxygenation? If unsuccessful in maintaining oxygenation, determine if the airway failed or go to next steps
- BMV or EGD predicted to be successful? If the Basic Mechanical Ventilation (BMV) or Endoscopic Guided Dilatation (EGD) is predicted to be successful, then move to next steps. If not then continue.
- Awake look/intubation successful? If awake looks/intubation was successful, go to next steps. If not continue with next procedure
- Physiology OK? If the physiology is okay (no apnea intolerance or rapid desaturation) then go to next steps, if not continue.
- RSI with double setup? If RSI (rapid sequence intubation) with dual setup was successful, go to next steps. If not then proceed to the next step.
- PIM or RSI? If the PIM (post-intubation management) or RSI (rapid sequence intubation) procedures are successful, then next step is to continue, or another attempt may be made
- Go to main algorithm: If the main algorithm is reached for completion, proceed to the next steps accordingly.
- One best attempt successful?: If a one best attempt was successful, then proceed to the next step (airway successful). If unsuccessful then proceed to the failed airway algorithm.
- Failed airway: This is a branch point indicating a failed airway management. Depending on the preceding steps, specific actions involving post-intubation management (PIM) or RSI with multiple setups may follow.
- Video laryngoscope, Flexible Laryngoscopy, Extraglottic device, Cricothyrotomy: these are possible procedures if other steps from the algorithm are unsuccessful.
