Study Notes
Tooth Preparation Stages
- Initial tooth preparation stage consists of 4 steps: outline form and initial depth, primary resistance form, primary retention form, and convenience form.
Final Tooth Preparation Stage
- Consists of 5 steps: removal of remaining infected dentin or old restorative material, pulp protection, secondary resistance and retention forms, finishing external walls, and final procedures.
Removal of Remaining Infected Dentin or Old Restorative Material
- Definition: Elimination of any infected carious tooth structure or faulty restorative material left in the tooth after initial tooth preparation.
- Conditions for removal: affecting esthetic result, compromising retention, radiographic evidence of caries, symptomatic tooth pulp, or periphery of remaining old restorative material is not intact.
Pulp Protection
- Definition: Placement of liners and bases to protect the pulp or aid pulpal recovery or both.
- A step in adapting the preparation for receiving the final restorative material.
Secondary Resistance and Retention Forms
- Mechanical preparation features (amalgam) and micromechanical treatments (composite).
- No bevels are placed at the cavosurface margin.
Finishing External Walls
- Design of cavosurface angle depends on the restorative material being used.
- 90-degree cavosurface angle produces maximal strength for amalgam and ceramic materials.
- Beveling is contraindicated except on the gingival floor of a Class II preparation when enamel is still present.
- Beveling enamel margins in composite preparations is indicated for larger restorations with increased retention needs and surface area for bonding.
Final Procedures
- Cleaning: freeing the preparation of visible debris with water and removing moisture with air.
- Disinfection: using Chlorhexidine (2 wt%) solutions.
- Inspection and desensitizing.
