Maximum Intercuspation
Maximum Intercuspation
The position where the upper and lower teeth fit together most tightly, often called the 'habitual bite'. It's independent of the jaw's position.
Centric Relation
Centric Relation
The position of the jaw where the condyles (the ends of the jaw bone) are in their most forward and upward position within the joint sockets (glenoid fossa). It's independent of tooth contact.

Study Notes
Denture Occlusal Schemes and Try-in Appointment
- The lecture covers complete denture occlusal schemes, including natural occlusion, balanced occlusion, unbalanced occlusion, and the wax try-in appointment.
- Natural occlusion, balanced occlusion, and unbalanced occlusion are different schemes used in natural teeth to distribute forces efficiently during chewing and prevent damage to teeth.
- Complete denture schemes aim for similar advantages, such as maintaining stability, improving chewing efficiency, preserving residual ridges, and improving patient comfort.
- Balanced occlusion limits tipping during functional movements, maximizing stability. This includes bilateral balanced, lingualized, and monoplane occlusion.
- Monoplane occlusion is characterized by flat, non-anatomic teeth without cusp inclines to eliminate horizontal forces and minimize lateral interferences. The scheme eliminates nonaxial forces on the denture.
- Lingualized occlusion features maxillary lingual cusps contacting mandibular occlusal surfaces. It maximizes stability and esthetics.
- Bilateral balanced occlusion involves simultaneous, evenly distributed contacts between upper and lower teeth on both working and non-working sides, ideal for complete dentures to enhance stability and prevent dislodgement.
- The try-in appointment verifies fit, function, esthetics, and phonetics. It helps to adapt or modify the denture.
Important Terms
- Maximum Intercuspation: The position where opposing teeth fit together in their most stable and tightest contact. Often referred to as the habitual bite or centric occlusion. This doesn't depend on condylar position.
- Centric Relation: A maxillomandibular relationship where the condyles are positioned in their most superior, anterior position within the glenoid fossa, resting against the thinnest, avascular portion of the articular disc. This position is repeatable and not dependent on tooth contact.
- Occlusion vs Articulation: Occlusion is the static relationship between maxillary and mandibular teeth when they are in contact. Articulation is the dynamic relationship during mandibular movements like lateral or protrusive.
- Protrusion: A mandibular movement where the lower jaw moves forward from centric relation or maximum intercuspation.
- Laterotrusion: A lateral movement of the mandible toward one side (working side) during mandibular function like chewing.
- Working Side: The side of the mandible that moves away from the midline during laterotrusion, with teeth often making contact.
- Non-Working Side (Balancing Side): The side of the mandible that moves towards the midline during laterotrusion, with minimal or no tooth contact.
Occlusal Schemes in Natural Teeth
- Mutually Protected Occlusion: Anterior teeth protect posterior teeth during excursive movements (lateral and protrusive) and posterior teeth protect anterior during centric occlusion.
- Unilateral Balanced Occlusion: Multiple teeth on the working side share the load during lateral movement, the non-working side has no contact.
- Bilateral Balanced Occlusion: Simultaneous, evenly distributed contacts occur between upper and lower teeth during all mandibular movements.
Christensen's Phenomena
- Christensen's phenomenon describes the separation of posterior teeth in both maxilla and mandible when the mandible moves into a forward (protrusive) position due to the downward and forward movement of the mandibular condyles.
Dentist Procedures and patient assessment
- The try-in appointment gives the first opportunity for dentist and patient to see the denture and decide whether to accept, modify or change.
- Check teeth color, mold, and best position during try-in.
- Assess alignment and any small improvements for patient.
- Check for tongue and lip support and adjustments.
- The try-in appointment has three possible conclusions: satisfaction, need for modification, or difficulty to satisfy both patient/dentist.
- The order of priority for information evaluation is fit, function, esthetics, and phonetics.
- Assess the wax-up for relaxed jaw posture, protrusive posturing, tooth characterization, gingival color, any other concerns, spouse/others satisfaction, and complete the details with other important oral evaluations, such as VDO.
- If both patient/dentist are satisfied, the denture can be processed. If not, posterior palatal seal must be considered for definitive impression when needed.
- Carving the posterior palatal seal can help compensate polymerization shrinkage, preserve peripheral seal, and blend into the mucosal contour.
- If the lower denture is not stable, single base stabilization should be used. If both denture bases are not stable, use double base stabilization.
- A clinical remount is needed if there's a major discrepancy, and teeth causing interference may need removal before bite recording. The bite can be recorded at the same or increased OVD.
- Consider the patient's age, ethnicity, complexion, and gingival pigmentation to determine the proper shade. Common shades include light pink, medium pink, dark pink, and pigmented shades.
