- The application for renewal of Authority to Operate CBW shall be supported by the following documents:
- Certified Copy of Amended Articles of Incorporation or Articles of Partnership, and by-laws, if applicable
- Updated General Information Sheet (GIS)
- Certified True Copy of Mayor's Permit
- Barangay Clearance
- Income Tax Return (ITR) and Latest Audited Financial Statement, if applicable
- Copy of the certificate of title to the real property or renewal of lease contract
- Warehouse location or vicinity map
- Updated list of machineries and equipment, if applicable
- Updated list of materials to be imported
- Formula of Manufacture, if applicable
- Affidavit of change of circumstances with supporting documents
Late Filing of Application for Renewal
- Non-filing within the prescribed period may cause the imposition of penalties, such as fines, suspension, or revocation of the Authority to Operate.
