Collision Reporting Centre (CRC) Criteria
- Minor collisions that do not meet "At-Scene" criteria require a report to be filed at a CRC within 48 hours, but emergency service workers have extended time to report.
- The process involves notifying the Communications Centre, exchanging documentation with the other party, and attending a CRC to complete a collision report.
Filing a Collision Report
- The driver must exchange documentation with the other party, including driver's license, vehicle permit, and insurance information.

- EAST DISTRICT: 39 Howden Rd. (Lawrence East and Birchmount), 416-808-4960
- Ontario Provincial Police operate out of 1 CRC facility in Toronto.
Reporting Collisions on 400 Series Highways
- For collisions on 400 series highways, crews must attend the OPP Highway Safety Division – Toronto facility.
Filing TFS Vehicle Collision Reports
- TFS Vehicle Collision Report (Form A) and TFS Vehicle Collision Report/Description of Events (Form B) must be completed, regardless of damage.
- Forms must be completed using a TFS station computer, with no handwritten reports accepted.
- The diagram portion of Form B must be completed using a straight edge and legend.
Incident Reporting and Documentation
