Routine Immunization Tally Sheet

• This document is a tally sheet for recording routine immunizations in health centers, clinics, and hospitals in Ethiopia.
• It tracks vaccinations given to children under one year old and those one year and older.
• Data is collected by region, zone/subcity/woreda, health facility name, start date and end date.
• The sheet lists various antigens (vaccines) like BCG, HepB, Pentavalent, OPV, IPV, PCV, Rota, Measles, and Td.
• For each antigen, counts of vaccinations are recorded separately for those under one year old and those one year and older.
• A total count is also recorded.
• Information on the type of session (static, outreach, mobile) is included.

Vaccine Types (Antigens)

• The sheet includes multiple vaccines, including:
  • BCG (Bacille Calmette-Guérin)
  • HepB (Hepatitis B)
  • Pentavalent (combining vaccines against diphtheria, tetanus, pertussis, polio, and Haemophilus influenza type b)
  • OPV (Oral Polio Vaccine)
  • IPV (Inactivated Polio Vaccine)
  • PCV (Pneumococcal Conjugate Vaccine)
  • Rota (Rotavirus Vaccine)
  • Measles
  • Td (tetanus and diphtheria vaccine)
• Each vaccine has separate columns for counts under 1 year and older than 1 year.

Data Collection

• Data is collected on the number of doses administered.
• The form tracks doses of various scheduled vaccines.
• Includes separate tally counts with vaccination timing information (e.g., HepB within 24 hours, HepB after 24 hours - 14 days).
• The form also includes a count or tally for "Fully immunized" and "Protected at Birth from NNT (PAB)".
  • Td all doses given (Td1-Td5) is listed as well.
