CCBHC Enrollment and Client Eligibility Process
- Clients cannot be enrolled in two CCBHCs if they have private insurance, and a copy of the insurance card must be made.
- A copy of the guardian’s state ID must be made.
- Client guarantors need to be set up in Go Health to set up insurance, including Medicaid 01AA-Adult Basic and 39AA-WRAP.
- Eligibility and Medicaid information should be inputted into Go Health, and manual eligibility for DMH and Medicaid should be chosen if the client has Medicaid.
- PINs need to be set for clients, guardians (if applicable), and treatment advocates in Go Health.
- The program name (e.g., School Based, Moderate Intensity Children, High Intensity Children/WRAP) should be put into Go Health.
- Various forms, including Communicable Disease Form, Intake Packet, Income Verification Form, Business Packet, Fee Worksheet, Fee Agreement, Medication and Allergies, and ROI forms, need to be completed and turned in.
- Clients wanting services in Tulsa Public Schools need to complete three School Forms.
- Various screeners, including TB Screen, PHQ-A, GAD-7, GAINS, Suicide Screen, Ohio Forms, SWYC, CATS Youth, and Caregiver, need to be completed.
- Health Risk Appraisal must be completed for all clients, including Health Risk Appraisal Youth for ages 0-17 and Health Risk Appraisal Young Adult for ages 18-25.
- The 21 CDC needs to be filed after 30 days of the first filing, and the second 21 CDC needs to be filed on the day of a PPS service.
