- Hypercalcemia of malignancy occurs when neoplastic cells secrete PTHrp
- PTHrp then binds to PTH receptors located on bone, kidney, and the gastrointestinal (GI) tract
- This binding stimulates osteoclastic bone resorption
- It also increases calcium reabsorption from the kidneys
- Calcium absorption from the GI tract increases
- Overall PTH decreases but PTHrp increases
- Humoral factors that increase calcium include PTHrp, cytokines, steroids, 1,25-dihydroxyvitamin D3, and prostaglandins
