CAMBRA Requirements from a Dentist
- Caries risk assessment should be done accounting for disease indicators, risk factors, and protective factors.
- Assign a caries risk level of low, moderate, high, or extreme.
- Make changes in the patient's lifestyle.
- Non-invasive therapies and minimally invasive restorative procedures should be carried out for tooth structure conservation.
- Recall and review should be part of the process.
Caries Imbalance
- Disease indicators such as white spots, restorations (less than 3 years), enamel lesions, and cavities/dentin lead to caries progression.

- Low salivary flow (xerostomia) or reduced buffering capacity.
- Limited or no fluoride exposure.
Extreme Risk
- Extreme risk is the same as high risk with severe xerostomia due to some systemic conditions like radiation therapy.
CAMBRA Treatment Recommendations Based on Risk Assessment Level
- Low Risk: OTC toothpaste with fluoride (1,000 to 1,100 ppm fluoride), 2x daily
- Moderate Risk:
- OTC toothpaste with fluoride (1,000 to 1,100 ppm fluoride), 2x daily
- OTC fluoride rinse (0.05% NaF), daily
- Xylitol candies or gums, 4x daily
- Alternative regimen: Xylitol candies or gums, 4x daily plus prescription 5,000 ppm fluoride toothpaste, 2x daily
- High Risk:
- Xylitol candies or gums, 4x daily
- Prescription 5,000 ppm fluoride toothpaste, 2x daily
- Chlorhexidine gluconate (0.12%) rinse 1x daily for 1 week, every month until the next periodic oral examination (POE), then reassess
- Fluoride varnish applied at first visit and at each POE/CAMBRA recall
- Extreme Risk:
- Xylitol candies or gums, 4x daily
- Prescription 5,000 ppm fluoride toothpaste, 2x daily
- Chlorhexidine gluconate (0.12%) rinse 1x daily for 1 week, every month until the next POE, then reassess
- Fluoride varnish applied at first visit and at each POE/CAMBRA recall
- Baking soda rinse, 2 tsp in 8 oz of water, 4x to 6x daily
