Report Forms
- NAVPERS 1610/5 is used for O7-O8 report submissions.
- NAVPERS 1610/2 is utilized for W2-O6 report submissions.

Promotion Limitations
- O1 (Ensign) and O2 (Lieutenant Junior Grade) cannot be promoted above "Promotable."
- O1 and O2 ranks are prohibited from receiving promotion recommendations higher than "Promotable."
Report Signatures
- Only civilians in GS9-GS12 positions may sign evaluations for E5 and below.
- E7 or E8 personnel are authorized to sign reports for E4 and below.
- The rating authority for E1-E4 reports is E6 rank or GS5 civilians.
- E7 or GS6 and above can rate E5-E6 personnel.

- Reporting for 10 to 90 days is acceptable, but such reports must be labeled "Not Observed."
- Reports covering fewer than 10 days require a Performance Information Memorandum (PIM).
PIM Specifics
- PIM serves to document brief performance periods and is not added to the Official Military Personnel File (OMPF).
- Suitable conditions for PIM include DUINS/TEMDUINS under 90 days, ACDU orders, or information needed for page 4 of Enlisted Qualifications History.
- PIM must be submitted within 15 days of completion of the assigned period.
Endorsements and Signing Procedures
- Flag Officer Endorsements are necessary when reporting senior is a captain reporting on other captains.
- A flag/general officer must endorse captain reports unless the report is classified as Not Observed.
Classification and Supplementary Reports
- Letter supplements can be classified up to Secret for reports.
- If a rater or senior rater is unavailable, document their status as “DETACHED, DEPLOYED, LEAVE” or "NONE AVAILABLE" if neither is present.
- A single individual may not sign multiple roles on the same report, and disagreements about the report's outcome cannot lead to separate submissions by raters.
