    Study Notes

    Bile

    • Secreted by the liver
    • About 600 - 1000 ml/day
    • Important for fat digestion and absorption by emulsification
    • Aids in the absorption of digested fat end products
    • Serves as a means for excretion of waste products
      • Bilirubin
      • Excess cholesterol

    Biliary Secretion

    • Two portions of liver secretion are added to the initial bile
    • The second portion is a watery solution of Na and HCO3 ions
    • This is secreted by epithelial cells lining the ductules and ducts
    • Increases the total quantity of bile by up to 100%
    • Stimulated by secretin, which causes the release of HCO3 ions

    Bile Storage

    • Gallbladder can hold 30-60 ml of bile
    • In 12 hours, 450 ml can be stored in the gallbladder
    • Water, sodium, chloride, and other electrolytes are absorbed through the gallbladder mucosa
    • Resulting bile is concentrated (bile salts, cholesterol, lecithin, and bilirubin)
    • Absorption is caused by active transport of Na through the gallbladder epithelium

    Gallbladder Emptying

    • Stimulated by fatty foods reaching the duodenum
    • Emptying occurs about 30 minutes after a meal
    • Also stimulated by acetylcholine-secreting nerve fibers from both the vagi and the intestinal enteric nervous system

    Bile Salts

    • Used for:
      • Creating small physical complexes with lipids called micelles
        • Detergent action on fat particles
        • Help in the absorption of fatty acids, monoglycerides, cholesterol, and other lipids
    • Emulsifying or detergent function breaks down fat globules into minute sizes

    Importance of Bile Salts

    • If no bile salts, up to 40% of ingested fats are lost in feces
    • Metabolic deficit because of nutrient loss

    Regulation of Biliary Secretion

    • The amount of cholesterol in bile is determined partly by the quantity of fat consumed
    • Liver cells synthesize cholesterol as a product of fat metabolism
    • People consuming a high-fat diet over a period of years are prone to developing gallstones
