Physical Status Modifiers
- P1: Normal, healthy patient.
- P2: Mild systemic disease.
- P3: Severe systemic disease.
- P4: Severe, life-threatening systemic disease.
- P5: Moribund; unlikely to survive without surgery.
- P6: Brain-dead organ donor.
HCPCS Anesthesia Modifiers
- AA: Anesthesiologist personally performed anesthesia.
- AD: Supervision of more than four concurrent procedures.
- QK: Medical direction of two to four concurrent procedures.
- QS: Monitored anesthesia care (MAC) – can be billed by a physician or qualified non-physician anesthetist.
- QX: Qualified non-physician anesthetist with physician medical direction.
- QY: Medical direction of one CRNA/AA by an anesthesiologist.
