Allen Cognitive Level Screen (ACLS-5)
- Used to identify cognitive levels in clients with Alzheimer's, dementia, TBI, CVA, and other disorders
- A quick assessment of functional cognition, processing capacities, learning potential, and performance abilities
- Identifies problems with functional cognition
- Three visual-motor tasks (leather lacing stitches) are used
- Assess attention, responsiveness to sensory/motor cues, understanding instructions, and observing client abilities/deficits
- Fosters feelings of success and usefulness
Types of Stitches
- Running Stitch: Hold leather, start at top left corner, push needle through, work clockwise, and complete 3 running stitches
- Whip Stitch: Start at top left corner, insert needle from front to back, leave 1.5-inch tail, secure under next stitch, and complete 3 whip stitches
- Single Cordovan Stitch: Start at top left corner (same as whip), push needle through next hole from front to back, pull lace until half-loop is formed, bring needle to front and push through loop, tighten stitch, complete three stitches
Administration of ACLS-5
- Running Stitch: Demonstrate stitch, demonstrate again as needed, three running stitches in consecutive holes are the completion criteria, score observation and go to next stitch.
- Whip Stitch: Demonstrate stitch, three correct whip stitches in consecutive holes (and recognizing/correcting errors) are the completion criteria, score observation and go to next stitch.
- Error 1 (Cross in Back): "Just do the best you can", note behavior and ability, go to Single Cordovan Stitch if successful.
- Error 2 (Twisted Lace): "I'm going to make a mistake to see if you can fix it!", make two errors, note behavior and score observations, go to next stitch.
Single Cordovan Stitch Administration
- Ask the client to engage in self-directed problem-solving to complete the stitch
- "Please make three stitches without me showing you how to make them. If you cannot figure it out, I will show you."
- Verbal cues = Max. 1, Demonstrations = Max. 2
- 3 correct single cordovan stitches in consecutive holes are the completion criteria
- Note and score observations
Ending ACLS-5
- Failure to complete 3 correction running or whip stitches is no longer a criterion for ending the assessment
- Criteria for determining ending the assessment: completing all 3 stitches, failure to correct error in single cordovan stitch after the second demonstration, refusal to perform or continue with single cordovan demonstration.
Scoring ACLS
- Detailed scoring criteria for running stitch, whip stitch, and cordovan stitch tasks
- Scores are based on observations of the patient's actions and abilities with specific criteria detailed in Tables.
Allen Cognitive Levels (ACLs)
- ACL 0: Coma, unresponsive
- ACL 1: Automatic and reflexive, minimal response to environment, 24-hour care, 1-second attention span, and total assistance.
- ACL 2: Postural and gross body movements, changes posture for comfort, helps caregiver with simple tasks (like rolling for diaper changes), attention span of ~2-3 minutes, maximum assistance with ADLs.
- ACL 3: Manual and repetitive actions, learn simple tasks, tactile cues, 30-minute attention span, moderate assistance with familiar ADLs (ACLS running stitch).
- ACL 4: Goal-directed and familiar activity, do simple tasks, visual cues and matching, unable to learn new tasks, routines are important, long attention span (hours), minimal assistance with ADLs (ACLS whip stitch); patient is able to go home from hospital and live alone (4.4-4.6).
- ACL 5: Exploratory and independent, new learning of concrete tasks, overt trial and error, frustration tolerance/impulse control, weeks-long attention span, standby assistance with ADLs, weekly supervision, and simple cordovan stitch.
- ACL 6: Planning and mental problem-solving, no cognitive disability, multiple-step tasks using analytic reasoning, independent with ADLs (cordovan stitches), "Plan to meet at 6"
