Acute Radiation Syndrome (ARS)
- ARS, or radiation sickness, is a condition in humans resulting from whole-body exposure to high doses of ionizing radiation over a short period.
- Data for this syndrome comes from studies of populations exposed to ionizing radiation, including atomic bomb survivors, Marshall Islanders, Chernobyl disaster victims, and radiation therapy patients.
- Early radiation responses are deterministic, meaning the severity increases with radiation dose and there is a dose threshold.
Stages of ARS
- ARS manifests in four stages:
- Prodromal (initial): Occurs within hours after a whole-body absorbed dose of 1 Gy (100 rads) or more. Symptoms, such as anorexia, nausea, and vomiting, are dose-related. This stage lasts from hours to a few days.
- LD50: The dose necessary to kill 50% of the exposed population.
- LD50/60: The dose required to kill 50% of the population in 60 days.
- Latent: This period lasts for about a week, with no visible symptoms. Recovery or lethal effects begin during this period.
- Manifest illness: Symptoms related to the hematopoietic, gastrointestinal, and cerebrovascular systems become evident. Severe cases can result in death.
- Recovery: Recovery may occur in some cases, particularly with lower doses. Late effects of radiation damage may be seen even after recovery.
- Prodromal (initial): Occurs within hours after a whole-body absorbed dose of 1 Gy (100 rads) or more. Symptoms, such as anorexia, nausea, and vomiting, are dose-related. This stage lasts from hours to a few days.
Hematopoietic Syndrome
- This form of ARS occurs from whole-body doses of 2.5 to 5 Gy (250 to 500 rads).
- The hematopoietic system, responsible for blood cell production, is the most radiosensitive system.
- Radiation exposure decreases the number of red blood cells, white blood cells, and platelets.
- Mild prodromal symptoms are initially experienced, followed by a decrease in circulating blood cell counts. Infection and hemorrhage are frequent causes of death.
Gastrointestinal Syndrome
- This syndrome emerges at a threshold dose of approximately 6 Gy (600 rads) and peaks after a dose of 10 Gy (1000 rads).
- Without medical support, death occurs within 3–10 days of exposure.
- Severe damage to the epithelial cells lining the gastrointestinal tract leads to dehydration, infection, and electrolyte imbalance.
Cerebrovascular Syndrome
- This syndrome occurs when the CNS and cardiovascular systems receive doses of at least 100 Gy (10,000 rads).
- High doses can cause death within hours or days.
- The prodromal stage is followed by a latent period where symptoms decrease or disappear.
- The manifest stage involves recurrence of symptoms with increased severity, potentially including death within minutes.
- The damage caused is to the blood vessels and capillaries in the brain, leading to fluid leakage and increased intracranial pressure. This causes tissue damage and ultimately failure of the CNS and cardiovascular systems.
Repair and Recovery
- Cells have inherent repair mechanisms (enzymes).
- Repair and recovery may occur from sublethal doses of ionizing radiation.
- After irradiation, surviving cells repopulate, potentially restoring organ function.
- The extent of damage determines an organ's recovery potential, and repeated injuries can have a cumulative effect on the body.
- Oxygenated cells are more damaged by low-LET radiation but recover more efficiently than hypoxic cells.
Additional Information
- About a third of Hiroshima's population was killed within a week from the bombing.
- Recovery may occur in 3 months or longer from exposure.
