- Epigastric hernia is also known as epigastric lipoma or fatty hernia of linea alba
- It involves herniation of extraperitoneal fat through the linea alba.
- Small pain and tenderness are common symptoms
- Pain is often referred to the epigastrium
- Can mimic a peptic ulcer
- Multiple in 20% of patients
- Midline in 80% of patients
- Treatment involves excision of preperitoneal tissue and closure of the defect
Spigelian Hernia
- Occurs through the spigelian fascia
- Medially, the rectus muscle
- Laterally, the semilunar line
- Located at or below the arcuate line
Interparietal Hernia
- The sac lies behind the external oblique aponeurosis
- Herniated through 2 muscles: the internal oblique (IO) and external oblique (EO).
- Localized pain and tenderness occur
- Diagnosis using USG or CT scans
- Treatment is operative closure of the fascial defect
- DASH technique involves intraoperative localization of the hernia prior to induction
