Study Notes

Abdominal Assessment Techniques

  • Four abdominal assessment techniques: Inspection, Auscultation, Percussion, and Palpation
  • These techniques are used to assess the abdomen and identify potential issues

Right Upper Quadrant (RUQ)

  • Includes the right lobe of the liver, gallbladder, and parts of the transverse and descending colon
  • Important structures to be aware of during assessment

Left Upper Quadrant (LUQ)

  • Includes the stomach, spleen, left lobe of the liver, body of the pancreas, left kidney and adrenal, and parts of the transverse and descending colon
  • Important structures to be aware of during assessment

Right Lower Quadrant (RLQ)

  • Includes parts of the descending colon, sigmoid colon, left ovary and tube, left ureter, and left spermatic cord
  • Important structures to be aware of during assessment

Subjective Data (Health History Questions)

  • Ask about:
    • Changes in appetite and weight
    • Difficulty swallowing or tolerating certain foods
    • Nausea, vomiting, indigestion, heartburn, or belching
    • Abdominal pain and its association with eating
    • Bowel habits and any recent changes
    • Laxative use and frequency

Abdominal Anatomy

  • Important structures to be aware of during assessment, including:
    • Descending colon, sigmoid colon, left ovary and tube, left ureter, and left spermatic cord
    • Abdominal muscles and their relaxation

Assessing the Abdomen

  • Light Palpation:
    • Perform light palpation to detect areas of tenderness and/or muscle guarding
    • Systematically explore all four quadrants
    • Ensure proper client positioning and warm hands
  • Palpating the Spleen:
    • Stand at the client's right side and reach over the abdomen with your left arm
    • Place your hand under the posterior lower ribs and pull up gently
    • Observe for masses, hernias, and other abnormalities

Performance Preparation

  • Pre-Assessment:
    • Check the doctor's order
    • Perform hand hygiene and apply gloves
    • Prepare equipment and introduce yourself to the client
    • Explain the procedure and obtain consent
  • Client Preparation:
    • Ask the client to empty their bladder
    • Ensure the room is warm and the client is comfortable
    • Instruct the client to remove clothes and put on a gown
    • Drape the client with sheets to expose the abdomen
    • Instruct the client to breathe through their mouth and take slow, deep breaths

Prepare for an abdominal physical examination by learning about observable masses, movement, and inquiring about the client's medical history. Practice assisting the client into a supine position with proper arm placement.
