SOGs

Additional Procedures and Guidelines
- EMS Captain ISO-360 or Medical personnel should be on the scene.
- Elevators: remove hazards below 3' and rescue above 3', with ladders extending 3'.
- Landing Zone Protocol: 120’x120’ LZ, ground crew 50' away at 5-7-12 o'clock, PPE required (Helmet, Eye/Ear Pro), maintain for 2 minutes.
- Comms: same Tac Channel for 2 helos in same LZ, different channels for different LZs, may use same as incident.
- Oxygen levels should be above 10% for proper operation.
- HCN presence smells like almonds at 1-5ppm, affecting 60-70% of people; OV=EO on a 5-gas meter.
- HCN Levels: 2.5-4.5 ppm, OV: 5-10 ppm, HS: 10-15 ppm, CO: 35-50 ppm, O2: 19.5-23.5 ppm, LEL: 10%-20% (Methane).
- Post-fire half-mask readings should show 20.9% O2 with only OV abnormal (Att. D 0% on all gases 20.9 O2).
- Gas Leak procedures: ERG 20-25, Orange pages 115-116; Propane range is 2.1-9.5%, NG range is 5.3-14%.
- Pipe sizes range from ½”-30”, mercaptan not added if 18” or above.
- At the Turnpike and Port West to the FPL Plant, 24” pipes carry 975 psi; residential pipes ½”-3/4”, non-residential 1”-4”.
- Ops Clamp up to 2” Sleeve, up to 5” Support Utility, NG company may need to mitigate poly above 2”.
- Tank sizes: BBQ 20lbs, Forklifts 33lbs, Commercial standing 100-420lbs, Residential underground 500-1,000lbs, HQ 30,000lbs.
- Fire Impingement: keep an above-ground minimum of 2.5” flowing at 200gpm, 1,000lb tank or larger ground monitor flowing at 500gpm.
- Bent and duct taped pipes: ½" - 3/4"
- Interior natural gas leak: vent naturally until 2% LEL, then use other means.
- Confirmed Exterior Leak: request BC/EMS, establish a hot zone (above 0%), use 2-in 2-out, water supply, deploy 1 ¾ fog minimum, lockout tag out, COMM center notifies gas company.
- Interior Gas Leak actions include: ISO, benchmarks, primary-secondary searches, ventilation.
- Documentation should include: all 4-gas readings before, during, and after; technician name, number, and who the scene turned over to; unit narratives.
- Water Rescue Teams consist of: primary swimmer, back-up swimmer, spotter (1/swimmer), and shore support.
- Requires an accountability officer if there are 2 or more swimmers.
- Minimum personnel to make entry: IC and 3 FD personnel possessing operational capability; or 3 FD personnel if to prevent imminent loss of life.
- Go Phases: I stabilize and II extract.

High-Rise SOG
- Tracking responsibilities: IC, Base, Lobby Control, Planning, Logs, Staging Ops, Div/ Group.
- SSG in first alarm 2; confined room fog at 30*.
- Open floor plan 100x100 or larger.
- Sprinkler: 150 PDP, Standpipe only: Hose lay +/- elevation, +25 psi SP FL.
- Combined system: 150-200psi.
- Ventilation static under 25 floors/ little flow above 25 floors: 3-6 mph ground to roof opening.
- Horizontal Ventilation: additional blowers needed if above 25 floors.
- Lexan Windows-Pebbles-In corners.
- High-rise problems: size/ complexity of interior ops, limited access, extended travel/ response time.
- Staging is 2 floors below and base is located outside. Area of Safe Refuge is 4 floors below the fire.
- 3 companies should be per line.
- Consider elevators if above 7 stories. Check the hoist every 5 stories.
- 2nd arriving companies should provide: water supply, back-up, RIC (asst. F/A, B/U or if not needed- RIC in stair on fire floor).
- The priority order for additional Command Staff: Fire Floor division, Lobby Control Unit Leader, Floor Above Division, ISO, Operations (2 floors below).
- Wildland arrival reports should include: acreage, access routes, direction of fire spread, exposures, types of vegetation.
- Communications need lookouts, escape routes, and safety zones. Rotations should occur every 30 minutes.
- Working Fire notifications go to Admin, DC, Investigator, and FPL.
- The max operating from a water tank is 200gpm.
- A relay engine is required if greater than 500'.
- Use tenders if more than 2 relay engines are used, consider a Water Supply Officer.
Fire Suppression and Rescue Tactics
- Structure Fire in one room: use 1 ¼" at 150gpm, Larger fires use 2.5” at 200gpm, Exposure Line use 2.5” at 200gpm, with exposure limited to 30'.
- Deck gun quick attack: 20-30 seconds.
- Transitional attack: 15 seconds.
- Book Cooling: 30-60 seconds.
- Softening: 30-90 seconds.
- In Large Area Search minimal standard of 5 personnel.
- A standard RIC team includes 2 or more firefighters, with a RIG minimum of 4.
- Small fires: less than 3,000 gallons
- Medium fires: 3,000-6,000 gallons
- Large fires: 6,000-15,000 gallons
- Very Large Fires: 15,000 gallons plus.
- NFPA 1407 pertains to RIC training; Chap 7 covers training, and 8 covers performance.
- Ventilation involves 3 principles: safety, containment, and property conservation.
- Ramfan should be used 4-10' from the door, with the exhaust opening about 1-2 times the size of the opening.
- 5 signs of pre-flashover: high heat, thick turbulent black smoke, rollover, uncontrolled fire, and auto ignition.
- Minimum number of personnel on a roof tasks include: Flat- 7-9-8 and Pitch Bay and Louver, needing 16sqft 4'x4'.
- Gear Cleaning adheres to NFPA 1851.
- Non-rated bridge should initiate rescues below the 20,000lbs Brush Truck.
- Brush Truck Equipment includes: 3” 800-1,000', 1 ¼ 200', and 2 SCBA's.
- 1 ¼ acre lot 200′. Single Family Home Flow: 250-500gpm. Total Gallons: 2,000-10,000.
- 4 methods of rural water supply: Relay, Nurse Tender, Modified Direct Pumping, and Dump Tank.
- Relay is generally preferred, Modified Direct Pumping yields 6,000 gallons; brush truck within 50', Nurse Tender when the tender arrives in first due and for a small fire, dump tank: 3,000-3,500 gallons, a minimum of 2 Tenders, and two tanks for flows over 300gpm.
- MVC full patient assessments include: greater than minor damage, no restraint, airbag, steering wheel damage, other occupant injury/ death, or abnormal appearance, behavior, or emotional state.
- MCI Levels: Level I: 5-10, II: 11-20, III: 21-100, IV: 101-1,000, V: Over 1,000, requiring automatic CISM for levels III or higher.
- FL Abuse line: 800-96-ABUSE and require LE and PBCVS for SART, and an EMS Captain. A vulnerable Adult is a minor. An adult is not wishing to report to LE or PBCVS without identifying info and MIH if the patient consents.
- Life-threatening refusal: SPO2 94, COPD <90, syncope, head injury w/LOC, fever over 100.5, severe pain, hypotension/ shock, stroke.
- 5 Hazmat levels: Awareness, Operations, Technician, Specialist, and IC.
- RIP-NOT: Recognize and Identify, Isolation, Protection, Notification.
Departmental Policies
- Cumulative Discipline: With 1 previous in the same alphabetical grouping the second will group together, while 2 separate groups remain different.
- Warnings/written reprimands drop in 12 months, and suspensions in 24 months.
- Discipline by ranks: Warning / Written Reprimand- all ranks; 1 day suspension- BC, greater than 1 day- DC or higher; Recommended demotion or dismissal- DC, Civilian Manager or higher; Dismissal or Demotion- Fire Rescue Administrator or acting FA.
- NOI to Firefighter within 10 days of Fire Rescue knowledge of incident.
- A special performance review occurs after 4 months (book 120 days), with potential loss of assignment or termination if still unacceptable.
- Minimum work time to have an annual performance review is 4 months.
- Probation reviews occur at the end of recruit academy, between 6-8-11 months.
- Special assignment over 6 months triggers a Special Performance Review.
- Promotion: 4 months and possible 2 month "employee performance review extension".
- Customer Complaint Procedures dictates contact must be made within 1 business day by DC or supervisor.
- A Customer Complaint File must be submitted on the intranet by DC within 30 days.
- A central clearing house handles complaints Ops in conjunction with Admin services.
- Inquiries are recorded on the Customer Inquiry Log and do need a Customer Complaint Checklist.
- A customer complaint form is initiated by an LT or higher and DCs are contacted verbally or a BC notified if unavailable.
- DC contacts Admin Secretary or FOO for a Customer Complaint Tracking Number and DCs may delegate fact-finding to BC if at the battalion level.
- Customer Complaint requires a Checklist and procedural document
- Also requires an Inquiry Field Guide. Ensure all actions summarized in the “actions taken” section of the Customer Complaint Form.
- The DC changes the status from open to submit, then assigns a higher-level supervisor.
- Harassment Immediate supervisor must assure behavior stopped, speak to persons involved, document (logbook, memo, etc.), issue a counseling form, contact next level supervisor for: developing a record of discriminatory activity, issuing discipline, and consulting IA.
- IA works with Diversity and Development section of HR.
- Complaints must be made within 300 days for PBC and US EEOC or 365 days for Florida Commission on Human Relations.
- Workplace violence encompasses 5 parts: Prohibited Weapons, Prohibited Behaviors, Identifiable Characteristics, Warning Signs, and Response to Acts or Threats of Violence.
- Identifiable Characteristics: 35-50 y/o male.
- For Non-imminent threats: notify the Deputy, who notifies IA and Union. For Imminent Threats: FOO/Comm Center notifies relevant EMS, BC, DC, Division, and Deputy, then IA and Union.
- IRE procedures involves the Firefighter must notify supervisor to BC by phone or face-to-face.
- The BC notifies DC by phone and follows up with e-mail by end of shift.
- The DC notifies the Division of Ops ASAP during normal hours, and the BC submits IRE with shared documentation by the end of the shift.
- DRE: Officer does the DRE form by the end of shift, and the BC notifies DC by e-mail.
- Uniform regulations: Nails and Admin Earrings ¼”, Heels and patch from seem to right shoulder 1”, nameplate from top of pocket, Chainsaw gap underside of bar 1/8”, Station logo on the Left Chest -4 ½”, Patch on sleeve -3 ¼”, Knit cap-55.
- The appearance Review Board: 1 from Chief, 1 from 2928, and 1 mutually agreed member.
- Emergency Leave contact: DC, and if Less than 6 hours: notify BC and then get approval by DC.
- AWOL: failure to notify lateness, notify absence, or report by 0800 hours.
- Firefighter loss of life or injury: requires two investigators.
- Recreational fires must be 50' from building, paved roads, and combustible liquid, with a max diameter of 36” and a max height of 30'.
- 7 Investigator call outs require: Criminal/ foul play, loss of life or ALS injury, FF loss of life or injury, accidental fires with substantial loss over $70,000 or 20% of structure, Juvenile, Explosive (under substantial loss call for them to determine response), or all other circumstances (cause undetermined, fire system activation, cause or potential cause of high value loss, overcrowding, or IC determines significant life safety issue).
- Donations given to BC, BC-DC, DC to Revenue Section of Finance Division (donation summary Workbook). Fire Rescue Administration Secretary notified, and a thank you letter of appreciation is sent to the donor.
- If personnel are sent home sick, the BC/Supervisor must notify DC by phone or e-mail. Personnel must receive 120 hours within 11/1.
- BCs are responsible for disincentives, must notify employee in writing or by e-mail and the DC. The DC notifies Wellness Coordinator when employee reaches EAP Referral level.
- If out for 3 consecutive shifts (5 for days), must receive clearance from the Occupational Health Clinic (OHC).
- In cases of on-duty injuries, the immediate supervisor obtains the Injury Tracking number from the intranet.
- If the injury involves PPE, it should be collected by the BC, and coordinated with the Training and Safety Division.
- If no Injury occurred, must coordinate by Supervisor-BC-DC-Training.
- The employee should be seen at the OHC within 48 hours.
- The state bureau must be notified within 4 hours if a firefighter is hospitalized more than 24 hours, suffers LODD, or there is a serious injury involving equipment failure.
- This contact should be made by the FOO, with authorization from the appropriate Deputy Chief.
- If out of work greater than 6 months, a medical exam and fitness are needed before returning.
- The exam may be held on days two weeks or longer to complete training and certifications.

- Use "responding" as a term when responding to a scene. Use "en route' for non emergency calls or anything besides hospital.

- Driving privileges require defensive training within 3 months and that an EVOC be completed every 3 years.
- Suspension of notifications must be given to end of business day.

- Remedial driver class is called for if accidents with 2 or more points acquired require in 90 days.
- The Apparatus Review Board requires: Manager at RM (non-voting Chair) Safety Specialist at RM, FD Safety Committee member, Training Division rep, 2928 rep that's a driver, (FF, LT, or Captain), BC, DC. Have a minimum number members to vote of 5 with tie breaker being chair, and a request in writing to Deputy of Operations within 15 days
- Jump suits: Allowed after 1600 hours. From 1800-0700 or BC discretion. No visitors: 2100-0700. Visitor time frame allowed 30 min (days 10 min.)

- Gear Cleaning adheres to NFPA 1851 Chap. 7 for cleaning. The ISP: turnaround will take be 36 hours. (No gear: chain of command to BC, BC to logs warehouse.
- CBRN exposure: retire immediately. Structural retire in 10 years. Proximity firefighting gear (ARFF): 5 years.
- Loss of government gear: Check to BOCC or payroll deduction over 3 months or 6 pay periods.
- 2-dimensional items are located in: SmartDraw. Company Inspections: Mobileyes. Inhalation Hazard facilities use: 302.
- Terms empty spaces- shell actual physical surveying- site survey areas in county with high risk potential for high damage and/ or high loss of life- Target Hazard.
- Pre-incident Plans: Annually. PIP required on commercial and residential with 5 or more units or common area, DC responsible for completion and BC responsible for verifying completeness.
- Shop hours: 0700-1800. NFPA 1911 problems OOS immediately.
- Professional Development max: 18 shifts: and inadequate forward to: division chief of Training
