PTA PROGRAM VA BEACH CAMPUS PTH 105 -- [Unit 7 Lab] -- [HO \#3] - [Functional Assessment Fall 2024] =========================================================================================================== **Learner Objectives**: D1. Describe the components of a functional assessment. D2. Instruct a patient and/or caregiver in the performance of functional mobility activities. D3. Document a functional assessment in the medical record. [Categories of Function]: 1. - - 2. - - - The following areas are addressed when performing a [physical functional assessment]: 1. 2. 3. 4. 5. 6. [Balance Scale] (one option used for assessing [static balance]) 1. 3+ Accepts slight challenges 4- Accepts minimal challenges 4 Accepts moderate challenges 4+ Accepts near maximal challenges Accepts maximal challenges -------------------------- [Balance Scale] (one option for assessing [dynamic balance]) \*This balance scale is slightly different than the one offered in the text -- this one contains more levels. [There are a variety of instruments used to assess a person's function]. Examples are: Dynamic Gait Index (DGI), Berg Balance scale, Oswestry, SPADI, Barthel Index, Katz Index of ADL, Functional Independence Measure (FIM), Sickness Impact Profile, SF-36, Outcome and Assessment Information Set (OASIS) Many rehab facilities use the [FIM (Functional Independence Measure) scale] for assessing a pt's function. PT addresses transfers, locomotion and stair negotiation using the following levels of assistance guide. 1. 2. 3. 4. 5. 6. 7. ### Lab Activities I. Assess your partner's sitting and standing balance (static and dynamic). Include A↔P and (R) ↔ (L) for static balance. 1. 2. II. Instruct your partner to perform the following activities. Your partner is a (R) CVA with (L) hemiparesis. Document your assessment (will go in the 'O' portion of your note b/c you are documenting their ability to perform a task) of your partner's ability to perform each of the following functional activities. 1. 2. 3. 4. 5. 6. 7. 8.
