inworm Infections8Chapter18 Joel Pinworm Infections= nocturnal peri anal/ perianal pruritus- nighttime Clinical Presentation: *symptoms= anal itching, sometimes no other symptoms, you can see the pinworm with a ashlight at night, or take some tape and the pinworm will come out and stick to it -Minor= asymptomatic -Major= abdominal pain, insomnia, restlessness to anorexia, diarrhea, intractable localizing itching -Severe- referred *Pinworm neurosis= psychological trauma of patient and parents that occur when worms are found near the a ected child anus; pt. Need to be be assured that pinworm infection are common & curable **HIGHLY contagious, can be airborne (eggs can be viable outside intestinal tract for 20 days) -non-drug measures are also essential Non-pharm: -wash hands before making food and after using toilet -encourage keeping ngernails short to prevent harboring eggs and autoinoculation -eggs are destroyed by sunlight and UV light -wash bedding, underwear, bedclothes, and towels of entire household in HOT water (killed in 131°F) -bathe daily in morning, shower preferred over baths -change clothes for several days daily after treatment -clean and vacuum (do NOT sweep) house daily for several days after treatment OTC-pharm: Pyrantel Pamoate (Reese’s Pinworm Medicine Liquid) MOA= stimulate release of Acetylcholine & inhibit cholinesterase & paralyze adult worm in GI tract & excrete in stool -ONLY FDA approved nonprescription med used -Indication= used in patient ages >2 whose weight at least 25 lbs -DOSE= 11 mg/kg as a single dose (MAX= 1 gram) -symptoms of pinworm infection that persist beyond 2 weeks after the initial dose, give 2nd dose, and if not working then follow up with a primary care provider -all members of household must be treated -side e ects: most common= involve the GI tract, nausea, vomiting, gastralgia, abdominal cramps, diarrhea, & anorexia Less common= HA, dizziness, drowsiness, insomnia
