Superscription. 2. Inscription. 3. Subscription. 4. Transcription. 5. Signature and date. 1- Superscription A. Full name, address, qualification, and telephone number of the prescriber. B. Registration number and date. C. Name, age, address, and gender of the patient. D. Date of writing the prescription. E. Diagnosis. F. The symbol Rx is an abbreviation of the Latin verb (recipe) take. 2- Inscription A. Name of the medicine. B. Strength of the medicine. C. Amount of the medicines. (frequency and duration). D. Time of taking the medicine. E. Pharmaceutical dosage form. 3- subscription It comprises direction from the physician to the pharmacist concerning the way of preparation of the substances to give the form and amount of the prescribed drug. 4- transcription It comprise directions to the patient for the use of the drug.
