Gouty arthritis ACUTE GOUTY ARTHRITIS There’s dense neutrophilic inflammatory infiltrates synovium & synovial membrane. MSU (Mono-Sodium Urate) crystals appear as as long, slender, needle shaped crystals within cytoplasm of macrophages & synovium. Synovial membrane is edematos, congested & show plasma cell, macrophages & lymphocytic infiltration. Chronic tophaceous arthritis Amputated big toe with thickened peri- articulations tissue showing white tophi involving the joint and soft tissues. There’s erosion of articular cartilage & destruction of juxta- articular bone. Tophi are the pathognomonic hallmark of gout. They are formed by large aggregations of urate crystals surrounded by an intense inflammatory reaction of foreign body giant cells. Chronic tophaceous arthritis MSU crystals form visible deposits (Tophi) within synovium. The synovium becomes hyperplastic, fibrotic, and thickened. There’s chronic inflammatory infiltrate in the form of (plasma cells, macrophages, Foreign Body inflammatory giant cells & scattered lymphocytes. Lesion is surrounded by fibrosis.
