Observe twelve (12) rights of medication administration. 3. Prepare the necessary materials: - IV Administration Set - IV fluids - IV canula - Medication card - Micropore - Cotton balls - Alcohol 4. Do hand washing/hand hygiene before and after all the procedure 5. Check the integrity of IV Solution, IV set and other devices. 6. Place IV Label on IVF Bottle duly signed by the IV Therapy Nurse RN who prepared it 7. Open IV administration set aseptically, close the roller clamp, and spike the infusate container aseptically following the infection control measures. 8. Fill drip chamber to at least half and prime it with IV fluid aseptically. 9. Expel air bubbles if any and put back the cover to the distal end of the IV set. 10. Explain the procedure to the patient. 11. Choose appropriate site, location, size and condition of the vein 12. Wear gloves only during venipuncture 13. Apply tourniquet 5 to 12 cm. (2-6 inches) above injection site. 14. Check for radial pulse below tourniquet 15. Prepare site with effective topical antiseptic according to hospital/institutional policy. (No touch technique) 16. Use the appropriate IV cannula utilizing the correct technique. 17. Insert catheter progressively until backflow is visualized. 18. Position the IV catheter parallel to the skin 19. Hold stylet stationary and slowly advance the catheter until the hub is 1mm to the puncture site. 20. Connect the infusion tubing of the prepared IVF aseptically. 21. Remove the tourniquet. 22. Open the clamp and regulate the flow rate. 23. Secure and apply the appropriate taping technique. 24. Tape a small loop of IV tubing for additional anchoring. Apply splint, if needed. 25. Calibrate the IVF bottle and regulate the flow of infusion. 26. Label on the IV tape near the IV site to indicate the date of insertion, type and gauge of IV catheter and countersign. ## **VIII. DISCONTINUING AN IV SOLUTION** 47. Check integrity of the IV solution 48. Place IV label on the IV bottle 49. Open, disinfect and connect the IV tubing into the solution bottle 50. Calibrate new IV bottle according to the duration of infusion as per prescription 51. Regulate the IV flow rate according to the prescribed infusion rate. Expel air bubbles, if evident 52. For discontinuing an IV solution, prepare necessary materials after checking the doctors order: - Bandage scissor - Cotton balls - Alcohol 53. Do hand washing/hand hygiene before and after the procedure 54. Close the roller clamp of the IV administration set. 55. Remove tapes utilizing appropriate techniques 56. Remove and inspect IV catheter for completeness. 57. Place dressing over the venipuncture site 58. Discard all waste materials including the IV cannula according to the Health Care Waste Management (DOH/DENR). 59. Do hand hygiene 60. Document time of discontinuance, status of insertion site, and integrity of IV catheter and hand-off accordingly ## **29. IV MEDICATION ADMINISTRATION** 29. For IV medication administration, check doctors order and medication labels. 30. Prepare the necessary materials and prepare the IV medication: - Medication card - Medication tray - Medication - Cotton balls - Alcohol or alcohol wipes 31. Confirm patency of the intravenous line. Assess for swelling, redness, phlebitis and other untoward signs. If evident, do not give the drug. 32. Check for skin test result of drug for IV push, drug-drug, drug-IV fluid incompatibility and dosage computation. 33. Check intravenous fluid labels. 34. Disinfect the rubber port of the IV Fluid using cotton balls with alcohol/ alcohol wipes and slowly incorporate the drug 35. For IV medication push through the IV port, disinfect the y-port using cotton balls with alcohol/alcohol wipes, kink the IV tube and slowly administer medication as prescribed. Release the kink to flush the medication. 36. For IV medication incorporation through the volumetric chamber, administer the prescribed drug into the volumetric chamber and add desired diluents aseptically. 37. Regulate flow rate accordingly 38. For IV medication push through the heparin-lock device, prepare the necessary materials: - Medication card - Medication tray - Medication - Cotton balls - Alcohol or alcohol wipes - heparin/heparinized syringe - NSS 39. Check the patency of the heparin lock 40. Clean the hub of the IV access port 41. Slowly administer the medicine prescribed. 42. Flush and lock the intravenous line. 43. Observe 5-10 mins for any drug interaction. 44. Discard sharp and other wastes according to Health Care Waste Management (DOH/DENR) and do handwashing 45. Document the procedure and hand-off accordingly ## **45. CHANGING THE IV SOLUTION** 45. For changing the IV solution, prepare necessary materials after checking the doctors order:
