Anatomy and blood supply to the Brain Neurosurgery Session I Outline 1- Surgical anatomy of the brain 2- Brain function of each part 2- Blood supply of the brain 4- Surgical fundamentals in brain surgery Lobes of the cerebrum Frontal lobe Motor strip Broca’s Motor It is an area which controls expressive language function, is located in the left frontal lobe Temporal lobe Wernick area Sensory Located in the temporal lobe Involved in the interpretation of speech Parietal lobe Awareness of Somatic sensation: Touch, pain and temperature Processing somatic sensation: Analysing, recognizing, memory of somatic sensation Occipital lobe Awareness of visual stimuli: Processing visual stimuli : Analyzing, recognizing and memorizing of visual stimuli Summary smell sensation and function surgical location Surgical Anatomy Limbic system function and location Brain stem function Cerebellum function anatomical surgical Blood Supply of the Brain Anterior cerebral Middle cerebral Posterior cerebral artery artery artery Main Arteries supplying the brain Carotid and vertebral arteries Two Brain supply systems Circle of Willis The Fusion of anterior and posterior circulation Circle of Willis Case example : A 71-year-old man was admitted to his local hospital with sudden onset aphasia and weakness in his lower limbs, which had occurred whilst playing golf. He reported no loss of consciousness, headache, nausea and/or vomiting, convulsions, mild bilateral lower limb weakness and altered behaviour and personality CT MRI ( exclude Mass ) Conservative treatment Case example : A 37-year-old Korean man, who was a non-smoker and previously healthy, experienced a sudden-onset right hemiparesis in April 2011. The patient was travelling in Guangzhou, China and was hospitalised within 4 hours of symptom onset CT / CT Angio MRI Surgical decompression Example case : A 40-year-old right-handed male presented to the local hospital with new onset right-sided paralysis and aphasia of 5-hour duration. CT Angio Treatment : 1- therapeutic window period for tissue plasminogen - activator (tPA) 2- Digital subtraction angiography demonstrated occlusion of the left P2 segment External carotid artery Areas of supply Venus drainage The venous drainage of the brain, i.e. the cerebrum, brainstem and cerebellum, is highly complex and specialised. Specific attention to the anatomy of the veins located in the brain is important for students, as unlike much of the rest of the body, venous drainage does not generally follow arterial supply in this region. The veins of the brain are divisided into superficial cerebral veins and internal cerebral veins, depending on whether they drain the superficial structures of the brain or the deep structures. Venous sinus thrombosis What is the function of the wernick area ? Interpreting written and spoken language Maintenance of equilibrium Articulation of spoken words Recognition of visual stimulus Thank you
